Through the University of Phoenix Office of Veterans Affairs, the University processes Montgomery GI Bill claims for veterans and dependents utilizing their VA educational benefits with University of Phoenix. In order for University of Phoenix to process VA entitlements, all paperwork must be properly completed and either faxed or mailed to the correct Office of Veterans Affairs within the University. Failure to include any part of your VA paperwork will delay the processing of your VA entitlements.

Additionally, your VA entitlements will be significantly delayed if you bypass the University of Phoenix Veterans Affairs Office and send your VA packet directly to the Federal VA Office. The Federal VA does not report or send any information to the University of Phoenix Veterans Affairs Office. Therefore, it's vital the University of Phoenix Veterans Affairs Office receives your VA packet in order to process your school enrollment dates to the Federal VA.

Montgomery (New) GI Bill - Chapter 30
Entered Active Duty after June 30, 1985. Completed at least 2 years continuous active service; contributed $1200 to the program; received a high school diploma or GED; and received an honorable discharge. For more information on the Montgomery GI Bill select benefits.

Vocational Rehabilitation - Chapter 31
Veterans who have been declared disabled by the Department of Veterans Affairs due to service related injury. - For more information on Vocational Rehabilitation contact your case manager.

Veterans Educational Assistance Program (VEAP) - Chapter 32
Entered the military service after January 1, 1977, contributed to the program, and did not contract with the Armed Forces under a delayed entry program. Benefits are dependent upon total contributions made and are calculated individually. No allowance is made for payment for dependents. Active duty service members may receive full VA educational entitlement under the VEAP program. Under specified conditions, participants under VEAP may be eligible to convert their entitlement to the New/Montgomery GI Bill. Dependents Educational Assistance - Chapter 35
This portion of 38 U.S.C. authorizes educational benefits to dependents and survivors of certain veterans who have serious disabilities as the result of their service, (100% disabled), or who have died of a service-connected cause, were a prisoner of war, or missing in action. Reserve GI BILL - Chapter 1606
Any person who enlists, re-enlists, or extends an enlistment in the Selective Reserve for not less than 6 years, in addition to any other period of obligation after June 30, 1985 and completed high school or GED, IADT, 180 days of service in the selective reserve and is satisfactorily participating in required training in the selective reserve. Activated Reservists After September 11, 2001 - Chapter 1607 (REAP)
Certain reservists who were activated after September 11, 2001 may either be eligible for education benefits or eligible for increased benefits. For more information on Chapter 1607 select on education benefits.

Applying for Benefits at University of Phoenix: (Top of Page)

If you are a Veteran:

Choose ONE of these two applications (22-1990 or 22-1995) based on your answer to the following question:

Have you used your VA education benefits in the past?

If NO, you will need VA form 22-1990 and the following paperwork:

• Completed Veteran Responsibility Letter.
• If you fall under Chapter 30/34 (Vietnam Era) benefits, you need to complete the VA form 21-686c.
• If you are on active duty, you need to have your Education Service Officer (ESO) sign your VA application.

If YES, you will need VA form 22-1995 and the following paperwork:

• Completed Veteran Responsibility Letter.
• If you are on active duty, you need to have your Education Service Officer (ESO) sign your VA 22-1995 VA application.
• Direct Deposit can be setup through the Federal VA's National Direct Deposit Program at 877. 838. 2778

If you are a Dependent or Survivor of a Veteran:

Choose ONE of these two applications (22-5490 or 22-5495) based on your answer to the following question:

As a Dependent or Survivor of a Veteran, have you used your VA education benefits in the past?

Federal VA Pay Rate Matrix for University of Phoenix Courses: (Top of Page)

The Federal VA Equivalency Time Table shows the training time and payment rate for non-standard length courses for undergrad students attending University of Phoenix. The Federal VA determines your "Full Time" status at University of Phoenix by this matrix.

Requesting a New DD-214: (Top of Page)

Your DD-214 is no longer required at the University of Phoenix Veterans Affairs Office. However, if you have misplaced or lost your original DD-214, you can request another copy from the National Archive and Records Administration for your own records.

TA/VA Top-Up: (Top of Page)

The school VA does not have the authorization to process Top-Up claims. All Top-Up applications go directly to the Regional VA Office at 888 .442 .4551 for processing. All Top-Up claims sent to the school VA office will be sent back to the student. Please review the official TA/VA Top-Up website for additional information.

Certificate of Eligibility or Veterans online APPlication (VONAPP): (Top of Page)

Students who have a Certificate of Eligibility (must specify University of Phoenix) or a VONAPP application, can submit one of these forms instead of a regular VA application. However, the supporting paperwork that is needed with the VA applications, is also needed with the Certificate of Eligibility and VONAPP. Failure to submit the supporting paperwork with your Certificate of Eligibility or VONAPP will result in a delay of your VA entitlements.

Enrollment Information:

• The Muskogee Regional VA Office is the only regional office that processes Axia College of University of Phoenix classes.
• All Axia College of University of Phoenix VA students are on Confirmed Enrollment Status: Students will receive VA payments AFTER the completion of the first week of each term.
• Please ask your Academic Advisor to "build out your schedule" in advance in order to receive your entitlements in a timely manner.
• Please have a valid e-mail address on file with Axia College of University of Phoenix - Veterans Affairs Office. You will receive an e-mail confirmation upon the certification of your enrollment terms to the Federal VA in Muskogee.
• Axia College of University of Phoenix VA students MUST take two terms simultaneously in order to receive the "full-time" entitlement rate.
• All students who are receiving either the Montgomery GI BILL Active duty or Selected Reserve MUST verify their enrollments monthly to receive payments. This verification can be done either by using the WAVE program or calling their automatic telephone system (IVR).

General Information:

• Axia College of University of Phoenix - Veterans Affairs Office does not evaluate prior military credits. Please contact your enrollment advisor.
• Your pay is retroactive back to your first day of class. Your VA entitlements will be coming directly to you from the Federal VA. The Federal VA will be able to tell you how many months of benefits you have left, as well as how much money you are entitled to per month.
• Axia College of University of Phoenix - Veterans Affairs Office does not participate in the "advanced payment" or "accelerated payment" programs.
• Please contact your school VA office if you plan on taking an "extended" break. There are only a certain amount of break days a veteran can take between classes in order to receive break pay. Please remember that a financial aid break and a VA break are not the same.
• The Federal VA pays entitlements by the month, not by how many terms that are taken per month by the student.
• Please contact the Federal VA six months before your delimiting date of your MGIB entitlements. You may be entitled to additional VA benefits through the Federal VA.
• The Axia College of University of Phoenix - Veterans Affairs Office has the responsibility to protect the rights of its students and to promptly investigate and resolve any formal grievances. To submit a formal grievance, please complete the University of Phoenix Formal Grievance Form and submit the paperwork to the VA Director of Axia College of University of Phoenix. Please fax all grievances to 602-366-3404.

Axia College of University of Phoenix - Veterans Affairs Office has a program to help individuals who are disabled, have been wounded or who have chronic health issues.
